You're listening to nature scope of the series of interviews with people of interest in northwest Oregon and southwest Washington I'm your host -- Douglas in previous generations -- was the -- to start families early you got out of high school maybe you went to college found the one got engaged got married not a house started a family often like my own parents before the age of 25 but these days many are choosing to wait to get their -- started take their time in finding someone they realistically would like to spend the -- -- life with. Or sometimes decide to go forth independently before settling down and having children well it's simple physiology that with many of us the older we get the more difficult it might be to actually have a child -- beyond that some have trouble even if they decide to take classic route and reproduce earlier in life well that's where the ever evolving world of reproductive medicine comes and to discuss this very topic I have with me today doctor Elizabeth Barbieri from organ reproductive medicine thanks for joining me today doctor how are you. -- thank you can -- you I'm wonderful and get things kicked off let's get our listeners a little bit of background on yourself and what you do for organ reproductive medicine. Oh well I can mention my name is Elizabeth Barbieri I am every protected and ecologist. What that means is that and that. Will be key light and can get additional training in the field of -- and a chronology of and ended up they made the majority of that work is now in the field of infertility and so I'm a gynecologist who specializes in the infertility. Look tactic called organ reproductive medicine which is a great group of doctors imagine embryology there's four commissioners total in our group. Could specialize in infertility and helping couples -- pregnancy. I say no reset infertility there about thirty times since we started yeah. Let's let's go over kind of a general explanation what is infertility mean. Everything is -- definition of infertility as published by the American Society for Reproductive Medicine -- about governing body of medicine for my field is in two women. 118 to 35. The inability to conceive after one year unprotected intercourse. Six. Now the caveat is that -- -- played in that definition right women have 35 and older. Infertility in the inability to conceive after six months to try. I say is actually in Mildred get the more difficult is going to be yeah. All right so -- 35 to that that's kind of the magic number that's when we start see fertility declines. -- but there's a very very settled. Declined. Around between 830 and thirty Q okay not noticeable I don't -- they don't have all the -- -- they're out there isn't ready. But added a very subtle decline and then into real agent 37 and 38 where they're Dumars Sharpton. The client I can't the competing and then when you get into your early forty becomes more difficult. -- all based on. That age of women -- and cannot do -- don't know what the quality of their eight are how good they are and how many do you have left. And -- talking to statistically because there are women of course who have had. Children in their forties you know that happens to my my sister and lost just to have her -- -- -- fourth child and she was like. 45. Yeah it happens. Yet you -- definitely different women out there. Who are able to compete at a later stage luckily the Obama don't have to come see me to -- but it broke -- at the thought that a women you trying to get pregnant that we -- And where did at the impact of age on fertility seemed to take control. So if someone is looking it you know did they make decision early on a life that they wanna have a child. In what would you say I mean certainly there are all the specifics of someone's individual life but. What's kind of a good age range just says chart track what would your suggestion. Still if you are in that position where use your relationship where you can get Greg writes. And you really wanna add to that a day I recommend you start trying by the aid. 3030 -- Even 34 that's reasonable you know -- and we can -- coming out with 3730 years also have been together for ten years. And maybe win. I -- started trying quote unquote a year ago. -- that's just an upset of yeah. How we drop the previous generation our parents have their kids early early and then you see them kind of burn out there early forty's and they they have the mid -- crisis -- They wish they had gotten some life in before they had kids and all that so we're all waiting a little bit longer. But which I completely understand and see but will win all the opportunity for women today. Cool wanna get their careers go to extended training went to Madison RM or. Email have a corporate have been anywhere they really want to achieve partnerships that it. Do you have to balance those desires with your desire to have the damage because. Sometimes there's a little bit of give and take. Right well I think you're looking -- 3032 that's a that's a reasonable range and -- have got your life started may be heading down the road you'd like to go down. Yes that being said -- have. If there's a thirty keep editorial women who are good are they should not be panic game at their running out of time. Ranked you know ought to go grab someone -- -- It shouldn't be gravity of -- with yeah. Yet take your time there -- there people who can help a little later on the -- winning winning would you say if you've been trying for a while it -- my wife and I for example we had a child urgent now twenty months old. But thank you but I -- 35 she was 38 and we were Tryon we've been trying for -- -- as -- we have -- had to get. But -- we've been trying for two years leading up to that. So we would you suggested you know if you had chatted with us during that time -- may be was trying to see if fertility specialist or how -- that determine when to someone see a specialist. Yet until I would be if you're 35 and older and you've been trying for six months and it hasn't worked out -- man. Please come and we're -- -- cute for you to do any treatment that you may not be comfortable little things but I think it's really important for people to be educated and about. What I can't get up competing are and what the potential roadblock that might be preventing them from competing. -- women age played a significant. Back there let there all the problems such as outcome to the number of stern met then predict. Make a really low amount of Sturm then no matter how do you argue made up till we get pregnant and you wouldn't 8%. Or if there's an anatomical problems such as my vehicle will be kids being blocked then there might be -- difficulty for the eight payment term to get there. And again regardless of age -- gonna cause a problem though that's what -- beat you up their investigation get all aspects of what it needed for construction. Do occur in our patient that we can come up with the treatment option and he uncover their problem that the patent were even aware -- -- products. Well we use the Murphy's law. -- method. But basically that was we both had a moment we re were switching careers at the same time and didn't have health insurance for about three or four months. And that's Oregon trying to protect that solitaire was just just having that that base not covered for three seconds then -- Now what is something that couples can do. If there if they are trying to conceive and it's maybe not happening right off the bat which I think is and seems to be more common now on the not. -- -- -- just because my from my friends in my world there are there a little bit older so this is not happen right off the bat. What can couples do to improve their chances of conceiving that mean you you -- kind of I don't know if they're old wives' tales being underweight glossy you're cutting out caffeine I've. I heard you know is the man starts working out because maybe it's a problem with there there are countless Australia -- Still as. Up good and the -- patient that couples can do our outlook how much common sense but in my mind I think they're great at what it have helped elect Barack built. There had to -- greatly -- there have been studies have linked to. Obesity especially in the woman Q I am not. Ideally and -- can -- get a many do not releasing an ache every month if you don't really can make you can get pregnant and he right Jake B -- exactly. -- that's one that's secondly. Even if you are on deleting it. It's steady to have show mad over obese women have -- people like summertime we get pregnant then normally live and and obese women who undergo fertility treatment. Also have -- champions of competing -- Whitlock is helpful for all aspects of our -- Partly to have an LC Bonnie having health buddy but admitted -- absolutely are you meeting motivation to this -- and trying to get pregnant. Also can especially important because obese women have more complications during pregnancy loses. The live public pressure and diabetes in pregnancy difficulty with delivery because they're all help. Taking prenatal vitamins the reason why we -- true this is that create -- vitamin. More so than when a woman might be getting her daily diet and this is extremely important and helping prevent -- tube defects that the problems with a spinal cord. And what I'm most people don't know except that early development of the organs are occurring in the first few weeks in united interpret pregnant. And the but I mean I bet you're pregnant -- association have not been drinking or smoking and I really -- for taking my vitamin tablet wanted to get an accurate picture trying to compete should be taking your vitamins. Lacking here alcohol intake is there's no safe level of drinking pregnancy. I had to stop smoking. Casinos Atlantic coast smoking causes lung cancer remember those correct those commercials -- -- show that I think Lundberg says the Blackwell Wright right are a big step. I think we have to get over. And you keep that -- -- -- I mean looking you're killing your -- you'd think yeah. Sounds like it basically this is going to be the vessel for developing a -- -- Make that the cleanest healthiest best we can. And I know what the -- couples can get kind of one to be more pro. In can make sure that I haven't integrated around the -- time of the month to right so I'm adding I'm tricky to current pad and notepad. Getting it at the right time to have a child and they eat and -- -- -- this evening -- really -- predictor kits. These are simple -- for lack of a better can you can buy your local. Pharmacy or target or you country Amazon.com. And he'd start Amman based on how long your menstrual cycle is you start monitoring it. But it's leaking -- That's simple once today and this. Can measure -- the hard -- your body and we'll let you know. When your most fertile period of time is because they'll showed that you are tabulated and that's the crucial time for which had -- -- it. App that works exactly so here are couple and you bought traveler one -- traveling a lot and here I think you might be missing at the Pentagon way to. -- come back into town this weekend night. Am I really have perfect. If you're just joining -- you're listening to -- your -- a series of interviews with people of interest to northwest Oregon and southwest Washington I'm your host -- Douglas and I'm talking today with doctor Elizabeth Barbieri from -- reproductive medicine about fertility all the ins and -- of that how to how to get some help how to know -- to try to have a kid when not to try to have a kid and what they can do to help out if you're having trouble so what are some of the signs. I gay you know you if you if you are trying. Any -- happening what are some of the signs that you're having possible fertility issues when should you go and spend speak with a specialist. We welcome TD that any point in the product that sometimes you wanna come earlier -- -- is America on the right track. But Kitty -- and women 35 and older. After a six month if you have not successfully competes. I would recommend coming in Jennifer complication Kelly we can educate you about what -- Bernama conception took her what might be happening in your case. And what continue to improve your chances in very high signal to the republic day at six month. Is that we know that women had that a 22% chance per month that getting pregnant it. -- you have pretty inefficient compared to the rest of the animal and animal kingdom sure. In women 35 and older the majority of the vast majority of couples who get pregnant we'll get pregnant by Nick Markakis. -- -- that curve and the plaque telling you can't getting pregnant after six months prior I can't do a lot. And we want to deal to improve their chances for you. You women apple from eight to 35 there we -- -- year trying I'd recommend coming in to be evaluated because. For example women who -- thirty years old 90% of couples will concede any. They're small percentage will not and that -- -- -- don't want to need help. But -- I'm 35 member Catholic 5% -- yeah. And then how do you go about dealing with if if you've got somebody who has been dealing with thanks a endometriosis is more fiber -- or anything like that that. How do you go about doing that. The confrontation will review all the different cabinet may -- it's and it's kind of getting pregnant if hit endometriosis. Often had to have had the surgery can diagnose that problem because they've been having McCain or another symptom I get to that endometriosis and we talk. That that -- that he individually over what would what treatment options are given -- special to -- -- You know the patient has severe and a major thank medically speaking to be -- -- -- -- -- depending on the extent that indeed it mayor Alvin and. And can you can you describe gonna -- and endometriosis and. Oh are very. You think is then. Act and you can disorderly and that and -- -- Which is medical term for the line of the -- actually -- -- uterine cavity outside the uterus and a completely. Inside the health and for the bill. Mean tiny people that -- uterine lining. Are picking on your overeat and under Philippine cute and I get pelvic sidewall and that they can be on -- -- -- -- your Blatter. Most commonly and to make you could present and tell that they really painful period of can maintain before their period. Something on the line. We you know that -- candidate -- associated with infertility and it's much harder for women with endometriosis can become pregnant right that he theories about why don't have to do is one. And images that can distort the public anatomy so the fallopian -- which are separate deal to move around and pick up a bit -- -- really -- from the overeat and get them. Can't help but I want to imagine if you can get I mean you plastered -- will -- you can't we gotta pick and Connecticut pocket coherent but candelabra. The same thing with your -- into the can't conduct it. But that theory that as -- -- -- infertility and that -- just as is often inflammatory to start the ability to creating. That it had a Mac -- pages all these and I like to speak of them evil humor. That's our topic for our -- and remember it. -- most couples if they have severe disease. I have that -- -- that there -- into the getting pregnant if they cannot compete on their own it'll probably witness in vitro fertilization. This is. Treatment option that we did some patients where he gives the women -- -- medication to make and produce many aches and remove the -- And fertilizer -- protect her partner in -- -- In a laboratory and watch those embryos grout and the removing The Hague and the sperm and the emirates from -- Evil humors and endometriosis the right and then we put them back inside the woman Jeter and. And -- -- is as pretty good success rate. I am very proud of a good argument predicament that we have excellent because it was excellent excellent advice. Sure sure and any -- listening and you you know you're having some trouble getting pregnant and you you immediately think like now. Oh well maybe it's endometriosis know you would not give you -- you'll be having some pain nearly something you should have been talking to -- OB GUN about the center of. Women know because there's symptomatic from pain but they -- and think -- just think about -- to be jettisoned that there is a subset of women who have been severe disease with very little pain. Really yeah. Well because I've had as the reason I've talked that in -- -- or my mother had severe case summit and then now so my my -- as well and both of them I mean varying levels of pain my mom had a pretty bad. But I had never heard of of not having symptoms. Yes -- can't -- -- women who don't have a good to have a safe. And yet the let me tell us. What you did knuckle -- -- they'll approach it is back. It and a woman is present because magic heading infertility I would describe enemy -- as a three possible reason for this happened but ultimately whether she hasn't Demetrius -- or not. Mike can recommendation will be that the. I say now I've heard of which are -- -- in vitro fertilization -- taken it down. -- also her taking names out to store them if you're going through a disease or you're having some sort of the problem or sometimes you know -- up and engaging for movies here but sometimes if it is a case where. You're young you store your eggs because you you wanna get their career started in you don't wanna -- not having things like. So that but he your preferred to -- in the name of the old great crowd -- a big -- or treating. Precinct will. -- at the art technology. Has gotten better and that the freedoms that we've. Offered a treating a patient and get the right now like most commonly offered to women. When it really technical can't let us look at most came out of there pretty kick agent of which are extremely important to treat their camp there and gave their life. It will often be toxic to their ovaries Inkster -- right exactly and talent -- just build. And we I think we've been fortunate enough to -- -- to patients if they find out about our treatment option and they will come to Seattle and we will. I stimulate them with art -- could grow there as many aches and possible and then removed the -- and treat them. Read before they start their -- treatment -- ideally they will then -- and Dakota can't achieve meant. The wind but I hope that they had -- through that -- that could potentially work what they have I think you see. How common is that you know. That. Commented right now and then there's it's gonna happen there's immediately needed to get. Many people who need it but I would depend on it I wonder if there's don't which -- -- -- would get -- talking but the command I feel at their patients who we are reach. We should know about it though. It that it comes I mean I'm glad to answer isn't as common as they could beat but I hope -- all -- -- who could potentially -- -- out of that nobody. I would completely he album. Once you've been a month for -- treating Bergkamp -- it's because. That's actually higher than I was not. -- -- you know we did it which means people are finding out about it and we're preserving fertility for several young women with the hope that they can use this in the future. The next round you get into the is what about -- treating her young you know a young woman -- been up Ph.D. program and thirty Q and you know it didn't involve -- in line and right though that you want to have children but doesn't need a good thing happening in her pretty able to your breath. And what about her -- her old. And that's kind of the -- and the technology becoming better and better that's the next. -- commit more viable option he. And -- when someone is trying to. -- -- know you know beaten Bure was organ reproductive medicine but I know there are a lot of organizations out there that it kind of do we do when. What are some of the criteria one must look for when choosing. One of these locations to deal with. And I still that's a very good question at. How do you know who don't have -- -- gonna help you caddie knows who you like. Because good and fertility treatments for the most part are not covered by insurance -- your spending a lot of money you really wanna make sure getting good service. And you wanna make sure that that the good people are gonna take care do you do with that for you not what's best for their academic and look good. I did that the very valuable question there is one. -- -- I'm called. RF AR key. Dot org it's entered the idea that the state reproductive technology no of all the critics that deal in vitro fertilization. Did you report their numbers their sixth straight to this body -- and they're. And then start publishes the data every year. Still if you're going to pick out there and they're not listed on minister registry that should bring. That's yes that's that's good mental depression -- And then patient should feel free to look at this and compare programs and then pick ERA you know in their general geographic area and he had a Big Apple they are from group to group. Also it's just a guy in his hand. Probably not a good bet exactly -- or appearance Southern California and I can comment is that the red day. Now does that especially bringing a lot -- mom. Is that something that happens opt into what we're -- has they help with the fertilization medicines and whatnot and me you know twins or triplets can happen. Adam -- actually happened. And that's what -- Fairly common in the -- Mamas and this is very strange case -- EG aberration -- and he'd ever hit America got the commission for that. Patient did not follow any of those -- -- grilled -- guidelines right advocated by -- governing body. But -- might have been the guy and a -- EX this is you don't come on out. Between are fairly common -- And he caught impatient about that hey you know what I see all organ predicament that we all are patiently to counsel them about what the treatment options are what the percent of having to wind is. -- -- treatment options and then we talk about the rate of having triplets because triplets are complicated pregnancy -- or not. Normal that the goal you comment to say no I wanna have three kids WAOK not -- want to -- this time exactly not all at once. And I think I can become difficult concept for patient to. Have been trying for so long and I don't get desperate need to have any pain that they take great three better than I -- to me everything exactly intricate and it -- -- -- One and I gotta walk without truly recognizing the potential dangers and complications. Of pregnancy. So where it eight Sunday I think that our education that we are conservative program. Debt has can be achieved a very good pregnancy rates with a very few. Higher order multiple but the temperature. Sure -- excellent. Well it sounds like you guys have a lot of success there that's organ reproductive medicine. I did and it's got to be a wonderful job because you're helping people who starts its new chapter in their life so what's your favorite thing about working with your patience there. You know I haven't. And look -- and they -- occurred about six months ago and it's not really having to her afford it except for three days just kinda apartment. And thank you really he pleased with how old friend and people are and how -- Open the patient where they really they come to EU and their interest and you. Would catch up. Personal and important time in their life -- and they may even compete they put all they continue and you have a responsibility to help them and when it worked. It would actually ever implement patient implement -- couldn't get -- it's wonderful political hack at the same time. -- I -- very hard when it's not working right and it -- very -- -- powerful that. You can be involved in the last part of the lake it well and try to come up with an option that will help them achieve their goal yeah. That's one of the things I love about Carlin as well he's had a people are very open seem to be a town largely full of realists. Think they they're not afraid to kind of play their cards on the table and deal that. -- that's wonderful. Welcome and believe it or not we've run out of time we would come -- to it but well done I like to thank you so much for everything you're doing its -- again sounds like a wonderful job and you're helping people out in getting -- next chapter started and I can only imagine how rewarding them must be.
