You're listening and -- go to a series of interviews with people of interest in northwest Oregon and southwest Washington I'm your host -- Douglas in previous generations it was the -- to start families early you got out of high school maybe went to college found the one -- got engaged got married about -- started a family often like my own parents before the age of 25 but these days many are choosing to wait to get their -- started take their time in finding someone they realistically would like to spend the -- your life with. Or sometimes decide to go -- independently before settling down and having children well it's simple physiology that with many of us the Hillary get the more difficult it might be to actually have a child -- -- beyond that some have trouble even if they decide to take classic route then reproduce earlier life well that's where the ever evolving world of reproductive medicine comes and to discuss this very topic I have with me today doctor Elizabeth Barbieri from -- reproductive medicine thanks for joining me today doctor how are you. I'm fine thank you can't tell you. I'm wonderful and get things kicked off let's get our listeners a little bit of background on yourself and what you do for organ reproductive medicine. I can't look at him mention my name is Elizabeth Barbieri I am every -- and a chronology and what that means is that -- and nine. D. J. White and -- -- get -- training in the field of protective and a chronology of and ended up they made the majority of that work is now in the field of infertility and so I'm about how to -- bachelor and the infertility. A look at a practice called organ reproductive medicine which is a great group of doctors and national member colleges there's four position total in our group. Could specialize in infertility and helping couples -- pregnancy. I say no reset infertility there about thirty times since we started yeah. Let's let's go over kind of a general explanation what is infertility mean. The definition of infertility and published by the American Society for Reproductive Medicine advocate about governing body of management for my field is in two women. -- the age of 35. The inability to conceive after one year -- unprotected intercourse. Cities. Now the caveat is live -- -- played in that definition right women -- 35 and older. And -- in the inability to conceive after six months and try. I say is actually in Mildred get the more difficult is going to be yeah. All right so 35 to that that's kind of the magic number that's when we start see fertility declines. -- and there are some very very battled. Declined. Around between 830 and 32 new okay not noticeable I don't -- say that have all the -- they're out there isn't ready. But added a very subtle decline and then if you really. -- 37 and 38 where there's -- -- sharp declined. -- the competing and then when you get into your early before he becomes more difficult. -- all based on. That age of women aches and they're not too brutal what the quality of their eight. And -- talking to statistically because there are women of course who have had. Children into their forties you know that happens to my my sister in -- just to have her -- -- their fourth child and she was like. 45. Yeah it happens. Yet you are definite they're different women out there. Who are able to compete at a later stage luckily those women don't have to come see me the right side it's where I -- -- -- -- trying to get pregnant that we see. And where did at the impact of age on fertility seemed to take control. So if someone is looking into you know if they make decision early on a life that they wanna have a child. -- know what would you say I mean certainly there are all the specifics of someone's individual life but. What's kind of a good age range just says -- -- what would your suggestion people. -- if you are in that position where use your relationship where you can get Greg writes. And you really want to add to that they I would recommend you start trying by the -- 3030 here. Even 34 that's reasonable you know -- and we had a couple -- menu at 3730 years also have been together for ten years. And me and and they started trying quote unquote a year ago. -- that's just an upset of yeah. How we drop the previous generation our parents have their kids really early in the and you see them kind of burn out there early forty's and -- to have a midlife crisis -- they. They wish they had gotten some life in before they had kids and all that so we're all waiting a little bit longer. A quick I completely understand and see the will -- all the opportunity for women today. Cool wanna get their careers go to extended training went -- the medicine or a model or. You don't have a corporate have been anywhere they really want to achieve partnerships that it. Do you have to balance those desires with your entire academic because. Sometimes there's a little bit of give and take complaint. Well I think you're looking -- 3032 that's a that's a reasonable range and have got your life started may be heading down the road you'd like to go down. That being said. If there's a thirty keep editorial women who are good are they should not be panic game at their running out of time. Ranked you know ought to go grab someone you met. It shouldn't be grab and have a kid with yet. Yet take your time there are there people who can help a little later on than their winning winning would you say if you've been trying for awhile like my my wife and I for example we had a child urgent now twenty months old. But thank you but -- is 35 she was 38 and we were Tryon we've been trying for a split as we have think we have to get. But -- we've been trying for two years leading up to that. So we would you -- -- you know if you had chatted with us during that time -- maybe it was time to see utility specialist or how how is that determined which -- someone see a specialist. Yet until I would be if you're 35 and older and you've been trying for six months and it hasn't worked out but he's coming again. Please come and we're never -- cute for you to do any treatment that you may not be comfortable little things but I think it's really important for people to be educated and about what they can't get up competing are and what the potential roadblock that might be preventing them from conceding. -- women that age played a significant. Back there. There are also problems such as outcome -- the number of -- that then predict right make a really low man Sturm then no matter how you and you are you may not built to get pregnant and you wouldn't -- percent. Or there's an anatomical problem such as my vehicle will be kids being blocked then there might be my difficulty for the -- -- -- -- to get there. And again regardless of age this -- gonna cause a problem because that's what we beat you up their investigation again all aspects of what it needed for construction. Do occur in our patient so that we can come up with that treatment option and had and he uncover their problems that the different even aware would've been problem. Well we use the Murphy's law. And method. But basically that was we both had a moment we re we're switching careers at the same time and didn't have health insurance for about three or four months. And that's Oregon trying to protect that solitaire was just just having that that base not covered for 32 and home. Now what is something that couples can do. If there if they are trying to conceive and it's maybe not happening right off the bat which I think is seems to be more common now on the not. And maybe this just because my from my friends in my world there are there a little bit older so this -- not happen right up to bat. What can couples do to improve their chances of conceiving that mean you you here kind of I don't know if they're old wives' tales being underweight glossy you're cutting out caffeine I've. I heard you know this demand starts working out -- maybe it's a problem with that there are countless Australia -- -- -- Up -- into that patient that -- can do our we will tell my comment but in my mind I think they're important -- what it have helped elect about killed there and did you rate weightless up there have been studies have linked to. -- -- Especially in the woman. Can you. I am not. Ideally -- American -- they met me you're not releasing an -- every month you don't really can make you can get pregnant can -- to be -- exactly. So that's one that's secondly -- even if you are on deleting the study can't show that over obese women have take a much from that time to get pregnant and then normally live and and obese women who undergo fertility treatment. Also have -- chances of competing bill. Whitlock is helpful for all aspects of her life you know partly to have an LC -- having healthy body but admitted -- -- time to really -- meeting motivation to lose weight and trying to get pregnant. Also can especially important because obese women have more complications during pregnancy loses. The live public pressure and diabetes and pregnancy difficulty would delivery because they're all help. Taking prenatal vitamins the reason why we -- true this is that create a vitamin. More so than what a woman might be getting her daily diet can be extremely important and helping prevent -- -- fact that the -- with a spinal cord. And what I'm most people don't know except that early development of the organs are occurring -- -- -- can united and character pregnant. And the bank I -- I beg your pregnant unit association have not been drinking and smoking and I ready for taking my vitamin -- wanna be to manipulate pictures trying to compete should be taking your vitamins. -- lacking here alcohol intake is there's no safe level of drinking pregnancy. I had to stop smoking. Casinos Atlantic -- smoking causes lung cancer remember those correct those commercials where they killed that -- Lundberg says the Blackwell Wright -- are expecting to have a -- over. Did you keep that vigilant and it has me looking you're killing earache if you'd think yeah. Sounds like it basically this is going to be the vessel for developing a child the value. Make that the cleanest healthiest -- you can. And I know what the -- couples can get kind of one to be more pro. -- can make sure that I haven't integrated around the great time of the month right -- I'm adding I'm thinking -- how to milk had. Getting it at the right time to have a child and at the way to do this evening ovulation predictor kits. These aren't simple -- for lack of a better can you can buy your local pharmacy or target or even country Amazon.com. And you start -- based on how long your menstrual cycle is do you start monitoring it but essentially picking on the -- Yeah that's simple once today and this. You can measure not the -- bottom level your body and we'll let you know. When your most fertile period of time is because they'll show that you are unrelated. And that is a crucial time for which have a little logo it. Exactly works exactly Tokyo couple and you bought traveler one -- traveling a -- and years I think you might be missing at the Pentagon way to. Brett come back into town this weekend I. Am I really have perfect. If you're just joining -- you're listening to -- -- -- a series of interviews with people of interest in northwest Oregon and southwest Washington I'm your host -- Douglas and I'm talking today with doctor Elizabeth Barbieri from -- reproductive medicine about fertility all the ins and -- of that how to how to get some help how to no wind to try to have a kid when not to try to have a kid and what they can do to help out if you're having trouble so what are some of the signs. I -- you know if you if you are trying. Any -- happening what are some of the signs that you're having possible fertility issues when should you go and spend speak with a specialist. We welcome TD that any point in the prospect sometimes you wanna come earlier this is another primary task. But traditionally. And women 35 and older. After the six month if you have not successfully compete with. I would recommend coming in -- complication Kelly we can educate you about what need to happen Bernama conception to -- what might be happening in your cape. And what continue to improve your chances in very bad signal to the recovery will be at six months. Is that we know that women has invented a 22% chance per month the getting pregnant. Did you have pretty inefficient compared to the rest of the animal and animal kingdom church in women 35 and older. The majority of the vast majority of couples who get pregnant we'll get pregnant by Nick Markakis. -- and that occurred and the plaque telling you can't getting pregnant after six months. -- I can't put a lot. And we want to deal to improve their chances for you. You women apple from eight to 35 there we today after a two year trying I'd recommend coming in to be evaluated because. -- for example women who are thirty year adult 90% of couples will compete -- Don't small percentage will not and that -- little people want and need help but -- -- -- 35 that number -- -- 65%. While yet. And then how do you go about dealing with if if you got somebody who has been dealing with like say. Endometriosis. And more fibroid -- anything like that how do you go about dealing with that. And -- a complication will review all the different test that may be it's an expert at getting pregnant if hit endometriosis. Often had to have had surgery to diagnose that problem because they've been having -- another symptom -- -- that have endometriosis and we talked. That that the that he individually over what would like treatment option to get an essential to getting pregnant. You know the patient has severe and it makes you think medically speaking to be gauged your speech for depending on the content that indeed in -- -- helping them. And can you can you describe financial and in Demetrius as -- Oh -- -- you think is then act and you couldn't order -- and -- am. Which is medical term for the line of the -- actually get from outside the uterine cavity out gladiator and a completely. Inside the health and for the bill. Mean tiny people that -- uterine lining. Are -- on your overeat and -- -- -- and I get held outside law and and they can be a nearing the second on your Blatter. Most commonly and images could present and public really painful period of -- -- maintained before their period. And something along like we you know that and images that were associated with infertility and it's much harder for women with endometriosis can become pregnant right. The -- period but why don't have to -- is one. And images that can distort the public anatomy -- -- fellow -- of which are supposed to -- up to move around and pick up a bit -- god really -- promote -- and -- -- Get a pelvic I want to imagine if you could get I mean you plastered it it will walk you can't we got to pick something out of pocket for -- -- -- -- -- -- The same thing with your -- into -- current DA. The pet theory that as to why Condit infertility and that and to -- just as is often inflammatory disorder. That would be creating the that a kind of -- is all these as a like I think of that evil humor. That topic for our aid department -- He'll most couples it they have severe disease. I have that addiction that there bench into the getting pregnant if they cannot compete on their -- probably witnessed in vitro fertilization. This is. Treatment option that we hadn't some patients where he give the women -- Amman a medication to make and produce many aches and remove the -- And fertilizer made protect her -- -- burned in a laboratory and watch those embryos to growl and -- -- removing The Hague and the sperm and the emirates from that. Evil humorous event and the chances right and then we put them back inside the woman Jeter and. Cindy and pregnant -- as a pretty good success rate. I am very proud of authority reproductive medicine because I think we have excellent because there's. Excellent I am I have and share share and ended the year listening and you you know you're having some trouble getting pregnant and you you immediately think like now. Oh well maybe it's endometriosis know you would not give you you'll be having some pain at least something you should have been talking to your BG UN about the center of. Women know because -- dramatic from pain but -- think you can think they'll just think about and make it isn't that there is a subset of women still have been severe disease with very little pain. Really. Well because I've had as the reason -- -- in a dangerous or my mother had severe case summit and then now also my mind and as well and both of them I mean varying levels of pain my mom had a pretty bad. But I had never heard of of not having symptoms. Yes I can't worry there women who don't have a good to have a -- And yet the let me tell. -- what you did not come and -- the approach it is back. It and a woman at present because magic having infertility -- describe enemy geoghan as a -- possible reason for this happened but ultimately whether he hasn't Demetrius did or not. Mike can recommendation will be that the. I say now I've heard of which are -- -- in vitro fertilization re taking it down. You know also -- take -- out to store them if you're going through a disease or you're having some sort of the problem or sometimes you know -- -- and engaging for movies here but sometimes if it is a case where. You're young you store your eggs because you you wanna get their career started in you don't wanna risk not having. Things like. So that appear to return to the -- name of the old -- -- -- -- -- our freedom. Praising the bill that. Bills at the art technology. Has gotten better terms of the freedoms that we've. Offered a treating a patient. And it. The -- now -- most commonly offered to women. When it really technical can't -- this -- the most came out of there pretty kick agent which are extremely important to treat their camp there and gave their life. It will often be toxic to their ovaries Inkster -- right exactly and talent -- -- build. And we I think we've been fortunate enough to -- to patient if they find out about our treatment option and they will come to -- -- and we will. I stimulate them with hormones -- ground there and many -- impossible and then removed the eight and treat them. Read before they start their cancer treatment -- ideally they were then -- and to go to cancer treatment. The wind I hope that they had to have proven -- that could potentially work once they survived against the. How common is that you know. -- Commented right now and then there's it's gonna happen there's going to be people who need it to get any people. I need it but I am I wonder if there's don't which is why would get that we're talking to African Amanda I feel at their patients who we are reach. We should know about it. Though -- that comes and I'm glad to answer isn't as common as they could beat but I hope that all these dude who could potentially be conservative and allowed that -- -- We completely he. When did you paint a month for -- treating bird can't -- it because. That's actually higher than I would not. -- you know we did which means people are fighting out about it -- we're preserving fertility treatment several young women without hope that they can music in the future. The next round you get into the is what about -- treating her young you know a young woman and even up Ph.D. program and thirty Palin. You know isn't involved -- an inline then right that you want to have children but doesn't need a good thing happening in her pretty able future threats. And what about her breathing her old. And that's kind of the -- -- the technology becoming better and better back the next. -- commit more viable option ES. And -- and when someone is trying to. -- say no you know beaten Bure was organ reproductive medicine but I know there are a lot of organizations out there that it kind of do we do it. What are some of the criteria one must look for when choosing. One of these locations to deal with. And I still don't have a good question and how do you know who don't have thanks so who can help you caddie knows who you like. Because good earnings fertility treatment to promote that are not covered by insurance year's spending a lot of money you really wanna make sure getting good service. And you want to make sure that that the good people are gonna take care we do expect for you not -- -- for their academic center look at. That the very valuable question. There is one website and called. RF ARE dot org it's entered the idea that the state reproductive technology the help of all the critics that deal in vitro fertilization they needed to report their numbers their sixth straight to this body -- and they're. And then start publishing the data every year. Still if you're going to -- out there and they're not listed on this registry. That kids. Mean that's that's yes that's that's good hands out of breath. Senate and then it. Patient should feel free to look at this and compare program and then pick ERA you know an account geographic area and he had a Big Apple they are from group to group. Also -- it's just a guy and his band. Probably not a good -- Exactly. Or here in Southern California and the -- -- -- -- -- -- -- Now does that especially bringing about a month. Is that something that happens -- -- where -- has to help with the fertilization medicines and whatnot and me you know twins or triplets can happen. Adam it actually happened. -- Fairly comment ideologues mom is -- isn't very strange case -- EG aberration -- and he'd ever hit America got that permission for that. Patients did not follow any of those. And -- a guideline right indicated by our governing body but -- might have been the guy than you -- it's you know come on out there. We are fairly common this. And be -- campaign about that at eighteen when I see all -- predicament that we all of our patiently to counsel them about what the treatment options are what the present at. Having -- is. Very treatment -- and and then we talk about the rate of having triplets. Because triplets are complicated pregnancy -- or not. Normal that the goal you comment to say no I wanna have three kids WAOK not I want us. This time exactly not all at once and I -- to become a difficult concept for patient to. Have been trying for so long and I don't get desperate he had any -- that they take great three better than I -- to me everything exactly and getting it took them. -- -- and I got to walk without truly recognizing the potential seniors and complications. Personal and important time in their lives and they -- even compete they cut other trends in UK and you have a responsibility to help them and when it worked. It would actually ever -- -- -- that patient a man positive pregnant again it's wonderful political hack at the same time. You know -- -- very hard when it's not working right and it got very skilled parable that. You can be involved in the part of the -- it well and try and come up with an option that will help them achieve their goal. And I guess that's one of the things I love about Carlin as well you send people are very open seems to be a town that largely full of realists.
